Community Support & Aged Care Services are designed to support older people who may require assistance with daily activities or who are experiencing health or mobility issues. Yuvan Care Provider can offer Community Support Services and Aged Care Services through a range of programs tailored to the needs of individuals and their families. Our team of dedicated and experienced professionals can provide services such as personal care, meal preparation, social activities, transport, nursing care, and support with everyday tasks. We can also offer respite care to give family carers a break when they need it. What we Offer

Older Persons Mental Health Services

Mental health services specifically tailored to older adults, who may have unique mental health needs due to ageing-related changes in physical and cognitive abilities, as well as social and economic factors. These services may include individual or group counselling, support groups, medication management, and care coordination.

Respite Services

Services that provide temporary relief to caregivers of individuals with disabilities or older adults. Respite services may include in-home or out-of-home respite care, such as day programs or short-term stays in residential care settings.

Home & Domestic Support Services

Services that help individuals with daily living tasks and household chores, such as cleaning, cooking, shopping, and transportation. These services are often aimed at older adults or people with disabilities who need assistance with tasks that may become more challenging due to physical limitations or health conditions.

Culturally and linguistically diverse (CALD) organisations

Organisations that cater to individuals from diverse cultural and linguistic backgrounds. These organisations may offer a range of services such as community events, language classes, cultural programs, and support services tailored to the specific needs of the community they serve.

Community Health Centres

Health care facilities that provide primary care, dental care, behavioural health services, and other health-related services to individuals and families, regardless of their ability to pay. Community health centres often serve individuals with complex health needs, including those with disabilities and older adults.

Transport Service

Services that provide transportation to individuals who may have difficulty accessing public transportation due to physical or cognitive disabilities, lack of access, or mobility challenges. These services may include wheelchair-accessible vehicles, door-to-door service, and assistance with boarding and disembarking. These services are often aimed at older adults or people with disabilities who may need transportation to medical appointments or community activities.

Application of Splints or Basic First Aid

Personal care supports can also include assistance with applying splints or providing basic first aid for any injuries sustained due to a participant's disability.
